2011 Affiliation and Insurance coverage will cease on January 31, 2012.  Therefore, for uninterrupted Affiliation and Insurance through ParaSport Ontario, it is essential that this form, along with any other documents and payment requested, be mailed to arrive in the PO office by no later than January 31, 2012.
